Complex Care Workshop: Addressing Social Needs in Dementia Care

This free day-long workshop-style conference will empower California based health care teams to assess for cognitive impairment, identify social needs of people with cognitive impairment, and learn skills and resources to manage their complex care.

Sessions are open to any member of a care team, i.e. clinicians, social workers, community health workers, and more.

The workshop will include sessions on how to conduct a cognitive health assessment and addressing common social needs of people with dementia and their caregivers, such as:

  • Avoiding conservatorship
  • Advance care planning
  • Medi-Cal and long-term care
  • And more

Participants of the workshop are eligible to receive 3 CME/MOC and CE credits or up to 3 hours of general MCLE credit. (MCLE Provider Number 9545)
