New Student Health Forms
We are so excited to welcome you to UC Law SF this fall! As a new student, we are doing everything we can to create a safe and healthy community for faculty, staff, and students. To that end, you are required to complete the following by July 30, 2023:
- Create a Carbon Health Account
- Complete your New Student Health Form & Upload to Carbon Health Account
- Comply with UC Law SF Vaccine Policy
New Student Health Form
As mandated by the University of California Office of the President, all University of California students must satisfy TB screening and vaccination requirements prior to the start of their first semester. To comply with this requirement, incoming UC Law SF students must receive all required vaccines, submit a copy of their vaccination records, and submit a completed New Student Health Form and HIPAA Authorization Form to the Carbon Health portal by July 30, 2023.
Students who fail to receive all required vaccinations and submit proof thereof by July 30, 2023 will have a registration hold placed on their record, preventing them from registering for Spring 2024 classes.
Create a Carbon Health Account
All new students, including international students, must create a UC Law SF-affiliated Carbon Health account by July 30, 2023. If you already have a Carbon Health account, you’ll need to create a new one, so that the services you use as a UC Law SF student are covered by your student health fees. (If you don’t create a UC Law SF-affiliated account, you will be personally charged for the services you receive and then we’ll need to help you resolve this problem on the back end.)
All admitted students will receive an email invitation from Carbon Health in June to create their Carbon Health Account, or can access this link upon receiving that email: https://carbonhealth.com/student-health/uc-law-sf, which will take you to a UC Law SF-specific registration page. Please follow the web-based instructions to create your account. If you are an international student, please note that you must use the link via your personal computer, and use the Carbon Health webpage, and NOT the Carbon Health app, in order to create your account while you are still living outside of the USA.
For now, you can skip the insurance information. But when your UC SHIP coverage begins in the fall or you’ve successfully waived out of UC SHIP, please revisit the insurance questions and provide updated information.
Complete the ”Assessment Tool.” You will be asked to provide general information about your healthcare needs, but you may skip the questions that don’t apply to you.
Complete the Privacy Agreement, which includes a Privacy Notice, Consent to Treatment, Consent to Telehealth, and Release of Healthcare Information.
