New Student Health Forms
We are so excited to welcome you to UC Law SF this spring! As a new student, we are doing everything we can to create a safe and healthy community for faculty, staff, and students. To that end, you are required to complete the following by December 1, 2026:
- Create a Carbon Health Account
- Complete your New Student Health Form & Upload to Carbon Health Account
- Comply with UC Law SF Vaccine Policy
New Student Health Forms
As mandated by the University of California Office of the President, all University of California students must satisfy TB screening and vaccination requirements prior to the start of their first semester.
To comply with this requirement, incoming UC Law SF students must:
- Receive all required vaccines and complete any required TB screening;
- Submit documentation of their vaccination records and TB screening history;
- Submit a completed New Student Health Form; and
- Submit a completed HIPAA Authorization Form.
All required documentation must be submitted to the Carbon Health portal by December 1, 2026.
IMPORTANT: Registration Hold
Students who fail to receive all required vaccinations and submit proof thereof by December 1, 2026 will have a registration hold placed on their record. This hold will prevent registration for Fall 2027 classes until the health requirements have been satisfied.
Religious and Health Exemption Requests:
Students seeking a religious exemption from required immunizations, or an exemption for health reasons, may submit these forms when creating your Carbon Health Account. There will be an option to submit the relevant exemption forms within the portal.
Create Your Carbon Health Account
All new students, including international students, must create a UC Law SF-affiliated Carbon Health account by December 1, 2026. If you already have a Carbon Health account, you’ll need to create a new one, so that eligible services are covered by your student health benefits. Please do not use an existing personal Carbon Health account for UC Law SF services. If you receive services through a non-UC Law SF account, you may be personally responsible for the charges and may need to work with UC Law SF to resolve the issue.
How to Create Your Account:
- Look for an Invitation from Carbon Health: All admitted students will receive an email invitation from Carbon Health in November with instructions for creating their UC Law SF-affiliated account.
- Register Using the UC Law SF Page: After receiving the invitation to create your Carbon Health Account, you can access the UC Law SF Carbon Health registration page: https://carbonhealth.com/student-health/uc-law-sf. Follow the web-based instructions to create your account. International students: If you are living outside the United States when you create your account, you must use the UC Law SF registration link from a personal computer in order to complete registration. Do not use the Carbon Health app to create your account while you are still living outside of the USA.
- Complete the Assessment Tool: The Assessment Tool asks for general information about your healthcare needs. You may skip any questions that don’t apply to you.
- Complete the Privacy Agreement: The Privacy Agreement includes a Privacy Notice, Consent to Treatment, Consent to Telehealth, and Release of Healthcare Information.
- Enter Your Insurance Information When Applicable: You may skip the insurance questions when you initially create your account. Once your UC SHIP coverage begins, or once you’ve successfully waived out of UC SHIP, please return to your account and update your insurance informationinformation.

Health Insurance and the Student Health Services Fee
The University of California requires all enrolled students to have health insurance coverage. Students are automatically enrolled in and billed for the UC Student Health Insurance Plan (UC SHIP). Students who can demonstrate that their health insurance meets the University of California’s minimum criteria may request a waiver from UC SHIP. The waiver application will open on November 15, 2026. The waiver application process is handled by our waiver administrator, Academic Health Plans (AHP).
IMPORTANT NOTE for MLS/CLS Students: All students who utilize UCSHIP will also be required to pay the Student HealthServices Fee of $965 and will have access to Carbon Health, our student health services provider. However, because of the nature of these programs, MLS/CLS students who successfully waive out of UCSHIP will not pay the Student Health Services Fee and will NOT have access to Carbon Health for health services. Students who successfully waive out of UCSHIP will not be able to pay a fee to utilize Carbon Health for health services. All students, including MLS/CLS students, will need to set up a UC Law SF-affiliated account with Carbon Health in order to have their New Student Health forms processed.
